​​The State System shall maintain an Incident Reporting System operated by an external vendor to provide independent collection of information related to suspected fraud, waste, or abuse. The reporting link is posted on the System website as well as the homepage of all System universities. The Office of Internal Audit and Risk Assessment, in consultation with the State System’s Office of Chief Counsel, shall be responsible for administration of the Incident Reporting System.
